Extremely rare occurances

I think that there should be a .01 chance to have an extremely rare occurance happen such as, triple gold from a gold rush, or 2 epic items in 1 drop, or a deathblow that kills an enemy instantly like the vorpreal axe in D&D. And a double epic drop would be too good but this is just a general idea.

I was thinking extreme chance stuff....like vorpal critical and death blows....maiming blows...magic bursts or magic overloads....massive gold drops....double epic or item drops....things that have a very slim chance of happening but are extraordinary when they do.
